[CO] Video release form for streaming on Twitch?

Hi there! I am live streaming a roller derby bout today via Twitch and I was wondering if you could create a notification that my announcer could say over the loudspeaker to the audience of the stream.

Ok, the party doing the recording has the burden of proving a proper disclaimer was made, which is why typically it would be signed upon entry or, for example, with ticket purchase as opposed to stating it as an announcement.

A logistical concern would be if someone did not hear it or if they arrived late. So the best thing to do is to draft an authorization and release and get actual signatures. Please let me know if you need further clarification.

Yes! That is part of our contract, and I got media release forms for the players, but I believe the audience also needs to be notified, so we need a template for that if possible.
