Ok. I would say you should be ok to just amend your documents/claim if the hearing is that far out. I do not see why you would need to cancel and reset the hearing here if it is that far out.
I cannot see any prejudice the other side could argue here of being informed of that now if you get that amendment filed, and they still have plenty of time to prepare for the hearing with that difference in the number. That is still 45 or so days away; that should be ample time with plenty of notice to the other side.

It should simply be what you filed, with the title of the document being "Amended ______" of whatever you filed; the same form(s).

Yes, you can use the same forms and add Amended to the title; there are no separate forms to amend.
