If you didn’t ask the court to change your name back as part of your divorce, you will generally need to file a Petition for Name Change (Georgia Code § 19-12-1). Sign the form in front of a notary public. Gather proof of identity, such as a valid Georgia driver’s license. Be prepared to pay filing and publication fees, which vary by county. Next, submit the completed forms to the Superior Court in the county where you reside.

Within seven days of filing, publish a notice of your name change petition in the official legal newspaper of your county once a week for four consecutive weeks. After the publication period and if no objections are filed, the court will schedule a hearing. Attend the hearing and provide any additional information the judge may require.

If the judge approves your petition, they will issue a court order legally changing your name. You can use this to update your name with various government agencies and institutions, including the Social Security Administration, Georgia Department of Driver Services, U.S. Passport Agency, banks, and other relevant entities.
