[AZ] [AZ] How do I find the docket and sequence number for my TOD form?

I went to a Maricopa County website a few days ago and someone was supposed to get back to me with the docket/sequence number but they haven’t. Now I don’t remember who or where that was.

Since you were waiting for a response from someone on the county website, you may have reached out to either the Recorder's Office or the Assessor's Office. If you need further clarification, you can contact them directly:

Maricopa County Recorder’s Office: (602) 506-3535
Maricopa County Assessor’s Office: (602) 506-3406

It should be available on the website; however, they have a duty to tell you in this case. You could also hire a local real estate agent to check for you, usually for around $100.

Is there anything else the lawyer should know before I connect you? Rest assured they’ll be able to help with your TOD form.

No, I think this is the last thing I need before filing. Maybe how do I file it after it’s notarized?

Got it. Are you a beneficiary?

No, my husband and I are setting up a TOD on our home putting our kids as beneficiaries.

So they can give me that info over the phone? Also, should it be on any refi paperwork I might have?

Correct! It probably isn't on the refi paperwork, but yes, the Recorder or Assessor can give it to you over the phone—and they should!

Thank you.
