[AZ] [AZ] How do I find the docket and sequence number for my TOD form?
Since you were waiting for a response from someone on the county website, you may have reached out to either the Recorder's Office or the Assessor's Office. If you need further clarification, you can contact them directly:
Maricopa County Recorder’s Office: (602) 506-3535
Maricopa County Assessor’s Office: (602) 506-3406
It should be available on the website; however, they have a duty to tell you in this case. You could also hire a local real estate agent to check for you, usually for around $100.

How do I find the docket and sequence number for the TOD form?
Lexi, Chatbot
I understand you're looking for the docket and sequence number for the TOD form. Have you already submitted the form to the appropriate office?
No, I’m filling out the form now.
Lexi, Chatbot
Which state are you located in? The process for obtaining the docket and sequence number may vary depending on your location.
Arizona.
Lexi, Chatbot
Is there anything else the lawyer should know before I connect you? Rest assured they’ll be able to help with your TOD form.
No, I think this is the last thing I need before filing. Maybe how do I file it after it’s notarized?
Lexi, Chatbot
Got it! Connecting you now.

Hello! Legal Eagle here. Have you already checked if any online resources or state websites provide this information for your TOD form?
I went to a Maricopa County website a few days ago and someone was supposed to get back to me with the docket/sequence number but they haven’t. Now I don’t remember who or where that was.
Legal Eagle
Got it. Are you a beneficiary?
No, my husband and I are setting up a TOD on our home putting our kids as beneficiaries.
Legal Eagle
Ok, that makes sense.

So they can give me that info over the phone? Also, should it be on any refi paperwork I might have?
Legal Eagle
Correct! It probably isn't on the refi paperwork, but yes, the Recorder or Assessor can give it to you over the phone—and they should!
Thank you.
Legal Eagle
For sure! Were there any details I missed? Any other concerns you need addressed?
That’s all, thank you.
Legal Eagle
You bet!
