We had an Advance Directive Form, which her previous Physician misplaced, unfortunately I can’t find the copy. Her current Physician is requesting a medical POA form, where her code status needs to be added.

In Michigan, it is called a Patient Advocate Designation (§ 700.5507). I cannot find an official form for this, but I have a link to a flyer from the state of Michigan website found here: DHS Pub 0161

That flyer refers to it as a durable power of attorney.
