How do I revoke power of attorney?

I need a form that will revoke my mother’s financial and medical power of attorney for my father. My father has Alzheimer’s, and my mom is his power of attorney. I was secondary. My mom’s health is declining, and I need to be able to take care of my dad if she is in the hospital.

Since your mother is still competent and willing to step down as your father’s primary agent under the POA, she can execute a formal Revocation of Power of Attorney document. This should:

  1. Clearly state her intent to revoke her authority as your father’s primary agent (financial and medical).
  2. Reference the original POA documents by date and title.
  3. Be signed and ideally notarized for recognition.

Once signed, provide copies to:

  • Financial institutions and healthcare providers.
  • Backup agents named in the POA (including yourself).
  • Keep a copy with the original POA documents.

If your mother is unable to sign later, guardianship proceedings may be necessary.
