[OH] [OH] How do I file a small estate affidavit for transferring stock?
You submit a small estate affidavit to the Montgomery County Probate Court. This is where you file for the simplified probate process for small estates (Ohio Revised Code § 2113.03). He will also need a certified death certificate. Has he contacted the holding company to see if they have their own packet for stock transfer?

Thank you for using this service! I’m reviewing your question. Have you already gathered all the necessary documents related to the estate?
I don’t know if anything else is required in order to get a small estate affidavit approved in Montgomery County, OH. I’m assisting my uncle. His wife passed away, and he found a stock certificate in her name. He needs it transferred to his name, and the holding company requires a small estate affidavit. There was no will. So all he has to do is submit that form with the death certificate, and that’s it? No other forms required?
