If you are 22 years old or older, Michigan law requires that you first undergo a criminal background check through the Michigan State Police (MI Comp L § 711.1(1)). You will need to have your fingerprints taken at a local police department, as well as pay a processing fee of $43.25. You can then file a Petition for Name Change with the court.

Some courts require that your fingerprints be submitted using this specific form: Michigan Applicant Fingerprint card (RI-008). Other courts will accept an electronic or "Live Scan" submission. Call the court where you are filing your Petition to find out which option the court will accept.
