- Co-applicants (if any)
- Authorization signature
Commercial Rental Application (For garages/business tenants) – should include:
- Business name, type, EIN
- Responsible party/owner’s name and contact
- Business address
- Years in operation
- Proof of income/revenue
- Trade references
- Intended use of premises
- Insurance certificate (if required)
- Authorization for credit/background check
- Signature
Accepting HUD Vouchers/Subsidies (Section 8, Shelter Assistance):
- Register with the Local Housing Authority (PHA) in Pennsylvania.
- Contact your county’s PHA and complete the Landlord Application. This typically includes:
- Proof of ownership
- Your LLC’s EIN and contact details
- Completed W-9
- Direct deposit form
- Lease agreement that matches HUD’s guidelines
- Pass a unit inspection (HUD will inspect for Housing Quality Standards).
- Sign a Housing Assistance Payments (HAP) contract with the Housing Authority.
