I am wanting to have my birth certificate corrected. My mother changed my name on Dec 23, 1976 to my stepfather and rapist of 10+ years. I want it corrected and I am wondering if I have to request a new name change or will an affidavit work.

You would have to file a petition for a name change to legally change it. Once granted by the judge, it is submitted to the Bureau of Vital Statistics, and you get an amended birth certificate.
