I'm looking for a power of attorney/medical form(s) which would allow myself or my wife to receive and/or transport medication for each other.

Getting a POA is relatively simple in this case. All you really need is:

  1. A description of the powers given to the person who will be the attorney-in-fact.
  2. The signature of the principal or someone who signs at the principal’s direction.
  3. The signature of at least two witnesses who are not going to be the POA (any two adults). You can also have a notary public sign the document in addition to or instead of the two witnesses. (Nevada Statute § 162A.790)

In your POA, add the following verbiage to state the powers related to medication:

"My Agent has the authority to obtain, manage, and administer any prescription or over-the-counter medications on my behalf, including speaking with doctors or pharmacies regarding my medications."

If you want to include the ability to refuse or consent to medications as well, you could add:

"My Agent may also consent to or refuse medications as needed for my care."

I used eForms to generate the general POA, but I wanted to add verbiage about medications to it—nothing related to advanced directives. We have that covered in a Trust.
