You would have to get the name change petition for a minor from the particular County Court where you would be filing the name change petition.
But this is a sample of what it would look like except for whatever County you're going to file it in: Minor Name Change Petition
You should be able to go on your local Courts website and find the same form with your courts information on it.

I’m trying to find the form I need.
I know that I need to get it notarized and turned I just can’t seem to find the form.

It doesn't look like that form is on the court website.
But since the forms are basically the same for every court, you could just modify one of the available ones to state Blount County and file it if the clerks don't have a blank template at the courthouse.
